The information contained in this application is complete and correct. I, the undersigned, give my authorization for the leadership of Creo Classical Academy to contact my references. I authorize any references listed to give you any information they may have regarding my character and fitness for work with minors at Creo Classical Academy. I release all such references from liability from any damage that may result from such evaluations to you, and I waive any right to inspect these references.
I authorize Creo Classical Academy to perform a criminal background check now and as needed in the future to update my records for arrests, convictions, or other information any law enforcement agency may have regarding me, and I release such information to Creo Classical Academy. I understand that the scope of the background check report may include but is not limited to the following areas: verification of social security number, current and previous residences, employment history, education background, character references, drug testing, civil and criminal history records from any criminal justice agency in any or all federal, state, and county jurisdictions; driving records, birth records, and any other public records. I further authorize any individual, company, firm, corporation, or public agency to divulge any and all information, verbal or written, pertaining to me, to the church or its agents. I further authorize the complete release of any records or data pertaining to me which the individual, company, firm, corporation, or public agency may have, to include information or data received from other sources.
Creo Classical Academy and its designated agents and representatives shall maintain all information received from this authorization in a confidential manner in order to protect the applicant’s personal information, including, but not limited to addresses, social security numbers, and dates of birth.
I release Creo Classical Academy and the above-mentioned references and agencies from any liability or damages resulting from the release of this information. I waive any present or future claims of privacy resulting from the release of this information for qualifications of volunteer work at Creo Classical Academy.
FAIR CREDIT REPORTING ACT (FCRA) NOTICE – BACKGROUND INVESTIGATION
In connection with your employment application with Creo Classical Academy Inc. (the “Company”), this notice is intended to inform you that a consumer report may be obtained on you from a consumer reporting agency for employment purposes. These purposes may include for hiring, retention, promotion or reassignment. The report may contain information about you relating to your criminal information or history, driving and/or motor vehicle records, verification of your education or employment history, social media or other background checks.
By signing below, you hereby authorize the obtaining of consumer reports by the Company at any time after receipt of this authorization and throughout the course of your employment.
You understand that the scope your authorization is not limited to the present and, if you are hired, will continue throughout the course of your employment, and allow the Company to conduct future screenings for retention, promotion or reassignment, as permitted by law and unless revoked by you in writing.
I hereby authorize the obtaining of consumer reports by the Company at any time after receipt of this authorization. To this end, I authorize any law enforcement agency, administrator, state or federal agency, institution, school or university (public or private), information service bureau, employer, or insurance company to furnish any and all background information requested by Protect My Ministry and/or the Company.
